This past year I was going through my records and noticed that my DD214 was missing a few medals. So I submitted a DD149 along with supporting statements and the award forms. Months later, I received a letter from the BCMR that said I was successful. Awesome. For USN/MC it is the Naval Board of COrrections and I believe you get issued a DD214-1
