Yeah, wouldn't it be just one silver oak leaf? Unless there is a "V" hiding out on the left side that I can't see.

No, this person is messed up. You don't need a 2nd ribbon until your tenth award. Assuming the individual has no VCR devices, which it doesn't look like, here's an example chart.

A sixth award should be noted with a silver oak leaf.
The only way that I see this makes sense is that there is a C or R device hidden by the lapel and that this is actually a fifth award.

It looks like another oakleaf sticking even after enlarging the photo I couldn't tell what kind of oakleaf.
AR 670-1 and DA PAM 670-1 should be able to address this question.

I want to know what ribbon is to the left of the Legion of Merit. It should only be the Distinguished Service Medal (based on it's color), or is it like the AAM and this guy is on his sixth LOM?

Is this a cropped picture of someone? Who? And if this person is a legit service member I don't think anyone is going to call him out on it.

There are instances where two ribbons is correct. AR670-1 addresses this.

It is SMA Dailey is you see his high res DA photo you can see he has a silver oak leaf and three bronze oak leaf.
