There was a privacy act one but that said on the form "this is not a form of release" or something like that. Is there not a release form? I thought HIPAA was waived when I enlist but it can't be if I never signed anything? IE, I had a prescription for Tylenol 3. My leadership did not know what prescription I was taking, just what the side effects were and how long I should be taking it. My kids' godmother was a combat medic and went ballistic on leaders who would even ASK the Soldier HIPAA protected questions. You do have to sign paperwork that allows MEPS to request your records for MEDICAL STAFF use and stuff like that. But your overall personal medical information is still protected from the general military population. Only certain limited details can be released to certain individuals. If you haven't signed one yet, they'll get it to you. I think I signed a new one every six months or so, and that was with me just being treated at TMCs, I never had an actual aid station. Both ex-husband's signed those forms even more often since their units had aid stations as well as the TMCs they went to. According to HIPAA, the way it works is that the law says your information is private, protected and will not go to any outside party not directly involved (eg. COs, other military personnel not in medical, etc) for as long as you shall live, so help you God. Specifically, information that could be used to identify a patient (discussing scenes or locations of an incident, specific medical conditions, birthdays/SSN/names, etc) is forbidden from being distributed - due to the power of the Internet, most healthcare providers are discouraged from posting or sharing any content related to their cases including but not limited to news articles or posts online, even on their personal profiles.

tl;dr Your information is still safe in Uncle Sam's capable hands, only medical personnel taking care of you will have access to pertinent past medical information. You sign a privacy act statement which states the information collected and the blood, urine, and any other tests and results found will only be shared with you, your recruitment team, your parents and your primary care team if necessary and will not be shared with any other party. That is so we can obtain Med records and or send you for more tests if necessary. It’s not really a true HIPAA form.
