The Department of Veterans’ Affairs said that it wrongly declared 115 veterans deceased and as a result revoked their benefits for a period of time.
The Washington Examiner first reported the admission, which was made by the agency in a letter to Rep. David Jolly (R., Fla.) after the lawmaker pressured the agency for information regarding several veterans in his district who had been incorrectly declared dead and deprived of their benefits.
The VA wrongly declared 115 veterans across the nation dead during the nine-month period between July 2014 and April 2015, the agency admitted in the letter. To prevent such errors in the future, the agency said that it is developing a policy that would involve using an automated system to declare veterans dead based on information from the Social Security Administration’s database.

Anyone besides me that sees a problem with this revocation? Why is it even necessary? What generates the death notification? Is there anything that the VA can NOT screw up?

COL Mikel Burroughs, sir, this story is evidence that we have a massive amount of people to track in computer software that in some operational situations should work and update automatically. The reality is, this does not happen in real time. Just like a bank will not process transactions after a certain time of the day to allow for processing and proper posting of debits and credits etc.
The VA has to keep track of veterans, widows, veterans returned to active duty due to reserves and guard. I get calls of veterans stuck in the middle of getting paid their disability and going to drill for two weeks or getting activated. During this transition they keep getting paid their disability when active duty pay starts. One would think that the DoD could have a switch in their systems of access permissions to switch a light in the us treasury to immediately stop payments. Because the veteran will lose out when his pay gets "readjusted" once "processing" takes effect.
It is not too outlandish that this can happen to veterans and I am not impressed with our ability to get contractors providing these software programs to work for our needs even if it means rival software giants working together to contribute to a more reliable software processing platform.
