Four deaths involving vehicle accidents on duty and the last one had no one wearing seat belts? How far up the chain of command does that one need to go? Sounds to me like a prime example of "if it ain't inspected, it ain't done".

"You expect what you inspect." That's the way I learned it. I was an NCO for my whole 20+ years except for the first year (-) more like eight months and I saw heads roll in Korea when we had an M113 go down during a river crossing, and then again when one of our Plt. members went souvenir hunting at an "administrative," bivouac site established by Battalion for field training. They had been briefed and warned not to go souvenir hunting. Just below the crest of that hill we were on was an old minefield and they found one. One troop lost a foot but was air-lifted to 121 Evac Hospital where he was tended to until he was transferred back to the states for discharge. Heads rolled on that one. In this case, Battalion should have inspected and found that minefield, marked it, and looked for a different admin site. In the case of the M113, safety measures enforced may have avoided loss of life. That is why you drive training until it becomes a habit or reflex. RIP
