Bowe Bergdahl - A Failure in Leadership?

I know I'm beating a dead horse here and we are still without most of the facts, but on Sunday morning, I was talking to one of my friends who is a Veteran and of course our conversation turned to Bergdahl and the trade and everything. Now Dennis served during the Cold War so we have a good time sharing different points of view and one thing that came up was, is this whole mess a failure in his leadership?

Think of this for a minute. A Soldier walks up to his NCO Support Channel asking questions about what would happen if he left with his sensitive items. Is seen mailing his stuff home in several boxes not yet half way through the deployment. Is making statements about "walking off" before and during the deployment and yet at least from the outside, it appears that no one with his chain of responsibility and/or chain of command did anything to reduce the possibility of Bergdahl doing what he did.

Is there anything that the leadership could have done short of putting eyes on Bergdahl 24x7? If a Soldier walked up to you saying what he said, what would do you? Can you see other Soldiers having the same idea during their next deployment? If Bergdahl is brought up on charges after the investigation, should his NCO Support Channel and Chain of Command be held responsible for not taking steps to reduce the possibility of him leaving the camp?
