Is it possible to get a 2-point waiver for my ST score to re-enlist into an MOS that I want? If so, how long would it take?

Looking to re enlist after a break in service. Need a 101 ST score for the job I want but my previous ASVAB has an ST score of 99. The job used to require a 91 ST score. Is a waiver possible? How long would it take?

Is a waiver possible? Yes. How long? You'll have to get that answer from a recruiter. I believe a 5-point waiver can be done locally by the commander, but the recruiter will have the answer about how far up the command channels it needs to go (which will give an indication of 'how long').
As Paul Harvey would say, Now for the rest of the story
First, you could always retake the ASVAB, and depending on how long your break in service was, you may be required to anyways (they are only good for two years, but don't expire while actively in the military).
Second, are you sure you can get into the MOS you want? Not from the point of view of qualifying, but you'll be a Prior Service candidate, and the business rules for those are different than a fresh recruit.
The last business rules* I saw for the Army were from a message in early 2020, and they can sometimes change frequently. You'll have to talk to a recruiter to see what the most current rules are, but these were the ones from back then (again, almost four years old, so very likely to have changed).
IF the business rules are still the same, the only MOSes that I see which require a ST of 101 are 35G and 68W. So ... talk to a recruiter. And since you'll have to talk to them about the current prior service business rules, you can use the opportunity to talk to him about retaking the ASVAB or submitting a waiver request. He'll have the most current information about submitting a waiver request and how long it will take.

Due to my little time on active duty since I'm prior service national guard, I don't have to adhere to the business rules. So shouldn't I be able to apply for a waiver? My recruiter is saying no but again, I don't entirely trust him. I go to DEP in and choose my job Thursday and I'll be speaking with the career counselors there should I just ask them?

SPC (Join to see) - I'm not sure where you got the "48 months of active duty time" from as Prior Service is categorized as having 180 days or more of active duty (if enlisting in the Regular Army) or if enlisting in the Reserve Component, 180 days of active duty or be MOSQ* (AR 601-210, Para 3-2).
However, you may not have to retake the ASVAB or get a waiver to qualify. You might have to educate your recruiter a bit about the qualifying scores for MOSes changing based on the WHEN the ASVAB was taken though.
If your ASVAB is from 2016, then you only need a 91 ST score. As AR 601-210 points out (para 3-6d), "When using previous test scores, counselors must follow MOS guidance listed in DA Pam 611 –21, as the ASVAB test has been revised several times and test score requirements will vary according to test date".
According to DA PAM 611-21*, the scores needed for a 68D (not 28D), Operating Room Specialist, are a minimum score of 91 in ST if the test was administered on/after 1 July 2004 and before 1 June 2019 (10-68D.b(7)(c) on page 393).

I had such a good time in the service I am a little jealous of this young man being early enough in his career to consider changes. Retired life is great but I still remember the first time I put on an army uniform and stood at attention at drill. Thirty years ago...

Yes, you can request a waiver for your ST ASVAB score to re-enlist. Since your score is close to the requirement (99 vs. 101), it's worth pursuing.
