Can I request a DD Form 363AF two years after PDRL as a SSgt Reservist?

Placed on TDRL in 2011 and retired (very informally) in 2016 by PDRL. No letters of appreciation or anything which is fine. But I would like to request something stating this other than a quick note from AFPC. Does anyone have any suggestions?
