Tiring to fill out the sponsor form. I cant find my units info to fill out the DA Form 5434. if anyone can help me with this thank you?

Tiring to fill out the sponsor form. I cant find my units info to fill out the DA Form 5434. if anyone can help me with this thank you. I have orders to 0002 CS REPL INBOUND (WEHS01) APO AP 96205, CAMP HUMPHREYS KOREA with a report date of 12-Dec-2019

Ah, my wife just got to Korea last weekend!
Your "unit/s info" should only consist of Company Commander name, rank, email, phone followed by the same for your 1SG and TASP Coordinator (Usually S1).
Are you saying you cant find that info for your gaining unit? Section 3 is completed by your sponsor and not you, so don't worry about that part.
