Does anybody have instructions for completing DA Form 4187, to request orders for continuing medical care, pursuant to Title 10?

Demobilized while in the middle of a medical issue. Would like to submit A Form 4187 to request being place back on orders for continued treatment and case management.

How in the Hell did they let you demob with medically issue? Get your paperwork to S1 ASAP, they should have assigned you to medical hold till it was foxed/sorted out.

That's what I'm tying to figure out. I went through the WTU process and and my MRPE was denied, even though the medical issue were confirm. The problem I have now is S1 doesn't know how to do the damn paperwork or where to send it.
