Does anyone have an example award nomination form for the Keeper of the Flame Award (Samuel Sharpe)?

I’m having a hard time with finding examples of what looks right. I have raw data to input but it needs to flow better for this to get a first time go at HQ.

It's a free form narrative. It's really riding on the O6 signature. If you want, you can PM me the narrative and I can give you some feedback. I've seen 1/2 page narratives and a page narrative. No more.
