I don't think there really is one. Each unit does their OpOrds differently, therefore the Reclama is ALSO different.
In essence, write a Reclama as you would an OpOrd, using the same basic template, but scaled WAY down, to include only the pertinent data about why filling the Order is not possible.
It has been 6 years since I was in a G3 writing these regularly, and I am retired, to boot. Otherwise I would hook you up when I got to work tomorrow. Sorry I am not MORE helpful, but hopefully this at least gets you started in the right direction/

SFC (Join to see) - I just looked at your profile. The G3 I was in way back when was YOUR G3 (USAICoE). I know for a fact that 111th and USAICoE G3s BOTH have reclama shells. I would be AMAZED if 304th did not have one kicking around their S3. I would strongly recommend reaching out to your S3 SGM. Even though you are likely reclama-ing a task from that same S3, they should still be able to give you the format to do it. (And I will also say that in a VERY few instances, a simple e-mail will suffice - especially if it is from a Company training room / Commander who does not technically have a staff.)
