I am trying to get everything together for my ets packet. Does anyone have an example or is there anything different for an ets leave form? Thank you.

Beyond your leave form, what other things are you preparing for your transition? Have you started working on your resume? Have you done research of what employment opportunities there are in the field of work you are looking in? Have you started the process of getting your VA physical done?
I do not ask these questions to be a jerk, just merely giving you somethings to also consider. We all know you'll be pulled in a thousand directions over the next few weeks. You'll be cleaning your TA50, clearing housing, ACAP, job searching, moving, all that fun stuff that comes with your transition so it's always good to be given a few reminders or some other targets you want to remain aware of.
Best of luck as you move into this new chapter of your life.

No I don't think any response is someone being a jerk, any advice or knowledge can be implemented.
I already have been accepted at a job. But I also have written a couple resumes just to be in the safe side. Scheduled my ETS physical and have appointments for the necessary lab work needed for the ets physical. I have finished all the ACAP classes and workshops and have my ETS packet taken care of and submitted. Everything else to m just waiting for my commander to approve to get that done. And I check up on that daily.
Thanks for looking out though, I appreciate when someone can give pointers when it comes to something like this.

SSG Steven Dowell - Well if you find yourself scratching your head, confused about something through your transition, please reach out. The last bit of advice I can give you is make a ton of copies of your DD214. Then, put a copy is a safe deposit box, another to a trusted family member and a yet another copy somewhere that you can access. You'll be handing them out like candy.

Very good advice. I'm definitely gonna have one framed as well so that's an extra backup.

Nothing different...your leave will end on your ETS. Depending on the amount of requested leave days, the battalion commander may need to be the approval authority.

Going to SFL and “PAYING ATTENTION” to the class should be a great way to figure out everything you need to do. Good luck.
