What are the components of a 17E reclassification packet?

Can't give you a definitive answer (will point you to the folks that can), but the requirements are laid on on the CCoE's website*. You should include proof of these with the 4187 you send up to HRC.
However, I suggest you contact the RA Reclassification Processing Team (RPT) for exact details.
