What goes into the packet for an officer to resign their commission?

I know that a memorandum from the officer is requested, but what DA Forms are needed?

DA Form 4187 with all the appropriate information filled out with the memorandum referenced and attached when submitted.

CPT(P) (Join to see) - Depends if the officer still has a MSO or not. The CG, HRC is the ultimate authority, but if the officer doesn't have an MSO, it could have been delegated down
