SFC Allan H. In my experience, I have never seen a requirement from big Army telling you what to have in your pt book, only local SOP and such. I might suggest having FM 7-22, a completed and signed CRM for the activities of that PT session, and the pt plan for that day. Anything else is probably unnecessary.
