Anyone have a template for the Humanitarian Service Medal?

My unit authorized the Humanitarian Service Medal for those doing Covid screening during a recent training event. We are all trying to find the template for the award and out Battalion S1 isn't exactly helping lol. Can anyone show me to the right direction

I found a template for hurricane HSM. You should be able to modify it for COVID. Similar information required.

Provide appropriate documentation IAW AR 600-8-22, para 2-24 proving they met the criteria of paragraph 2-24c and d. More info on HSM in MILPER message 20-236

Service awards don't have certificates. The orders are what get uploaded to IPERMS . You can find templates, but there just unofficial mockups that some just made up.

Hey, granted I retired in 2017 but doesn’t an operation have to be authorized at a much higher level than your unit to qualify for the HSM? That’s why there’s a specific list of approved ops and C-19 isn’t on it as far as I know.

Please correct me if I’m wrong but that’s how it was when I got my HSM.

1SG Joseph Yorski, MHS - Indeed, thank you 1SG. I provided that link because I wanted to stress to him that you have to be in a named operation ISO a humanitarian effort. Just serving during COVID-19 doesn't net you a HSM. I'm worried he's misinterpreting the announcements.
