Anyone here fill out a DA form 3284-R "Applicant's Statement of Name Change" while enlisting between 1 Nov. 1967 and ? Sept. 1976?

If so, are you now having a problem with your state not wanting to honor it (combined with your DD 214) as a legitimate name change? Anyone know if the DA form 3284-R was a stand-alone form; or, a pricess starting form?
Anyone have any idea how to get copies of USAREC notices for AR 601-210 leading up to the implementation of that form, in 1967; and/or, its being made obsolete in 1976? I keep getting looped back to HRC in Ft. Knox; but, even though they've tried to be helpful, it's been nothing but dead-ends.
