i want to submit my refrad packet but it was signed late because my BN commander was avoiding it for a month until i filed an IG complaint he signed it the very next day (14th) and didnt send it back to me until yesterday so i want to write a memo addressing the COL and HRC why im submitting it late but i have no idea how to write or what to say... does anyone care to help me with this task ?

Just stick to the facts, be tactful about what happened, and hope that they are understanding. If you just keep it simple and apologize for it being late for the above stated reasons, then that should be sufficient. Avoid blaming your BN Commander, but mention that they signed it much later than you had anticipated and only just received it back on the 14th.
