Does anyone have an example of the UCMJ memo for the recruiting packet?

I am filling out a volunteer for recruiting. I am to the part where I need a memo for UCMJ action I received 10 years ago. Does anyone have an example of this memo?

I realize this thread is 6 months old. However, if still pertinent and for anyone else who may need the exmaple memo for previous UCMJ actions, I've attached the example I have.
