I enlisted into the Army active duty two years ago but I wasn't in a good place in my life and got discharged on chapter 5 11. I was told I would be able to aquire a waiver to reenlist, how should I go about aquireing a waiver?

You need to consult with a recruiter.
Considering that separation is based on a preexisting medical condition, you may want to also review DOD Instruction 6130.03 Medical Standards for Appointment, Enlistment... as you may likely require a medical waiver, too.
