I am guessing you can download them from my MEDPROS, but every time I try to go on the site it is unable to connect, I am using my personal computer with CAC reader, do I need to use a GOV't computer to access or am I looking in the wrong spot for these forms?

Purely as a suggestion, if the group has a toll free or commercial phone, call them directly, get a supervisor and/or !manager, explain the problem, give them your email address, and ask of they can send you the stuff as PDFs, preferably both fillable and saveable versions, if they've got them, as I'd said, just a thought, with stuff like that, I always go with Alexamder Graham Bell, that's why phones were the most valuable patent ever issued, don't send them emails, call them directly, if you even have to ask a switchboard operator to get you someone higher, generally, that's what my wife and I do whenever we've got aggravation of that type, hunest, if you've got any thoughts or might want to chat at all, whenever you'd want, no rush, whenever convenient, I'd be eager to know if youre able to get anywhere doing what I'd suggested, as well, if at all possible, I'd be most curious, no biggie, whenever you'd be able, hope that was all of some help, at least....

I follow...if you didn't ask for a supervisor or !amager when you called, possibly try that, and ask if someone higher might have more info...another thought is, if you have a clinical facility at your installation, ask to chat with a senior !ed admin at it, explain, and see if they can either get you the forms, or possibly ask them to !make some calls...clearly, I know nothing about MEDPROS, per se, however, I should certainly think if you can call TRICARE, and ask on their toll free phone line, for a supervisor and/or manager, I'd be quite surprised if someone on that phone line wouldn't have the info, just a thought I'd had just now, you know?

When I typed in TRICARE toll free number on Google just now and scrolled down, one link I hit under a list of questions had sevl toll free phones listed...I'm quite certain a supervisor and/or manager there, as I'd said, should certainly be able to get you a toll free direct to MEDPROS, or maybe an address or possibly a commercial phone as well, I would think....

Easy answer: your local medical treatment facility, MTF, records department will gladly get them for you. They may be called records or PAD, Patient Administration Department, or something along those lines.
MEDPROS will have your immunization records but it will not have any real medical documents, just the results from your exams.
Whenever you get the joy of experiencing the new MHS Genesis, you can get a couple items from their patient portal.
The hardest record to retrieve will be dental. All of your other medical exams are in AHLTA/JLV. Dental is in its own program.
My recommendation is going to your MTF's records department. You may need to start with your local aid station and see what they can do; otherwise, larger facilities can do it.

Thank you sgt, I did as you instructed and got my forms immediately. Fortunately I didn’t need dental after all.
