If one of my soldiers loses their ACH, what form needs to be filled out to get them a new ACH, along with paying for it?

Long story short. A battle of mine lost his ACH. He is being told he has to buy a new one but my peers can't figure out the form. I am asking for help to try and ease the process.

He will need to go CIF in order to draw a new one. They will send him to supply for a statement of charges. A new ACH costs around $350, I believe.

SGT (Join to see) - CIF can do the transaction on site. They can let you swipe a card unless it is one of the prehistoric ones.

SGT (Join to see) - you'll need to initiate the statement if charges in your unit supply room. Also call the customer service desk first and see if they need you to process the Statement of Charges at Finance or if they can draft the funds. Fort Carson has a cashier cage at CIF.
