I got out in 2013, and I am looking to get back in. I am trying to get all my medical records in order. I had a cyst removed by a surgeon, and my medical records include all of that data. I also had it lanced/drained at sick call, and reported it on my PHA. I noticed that there is no record of me reporting it to sick call.

So, I guess my question is: Are PHA records stored and retained? If the record of me reporting the issue during my PHA aren't listed in my medical records (the ones that they printed out and sent me when I ETS'd) is it safe to assume that the record doesn't exist?
Can the docs at MEPS access these records?

Just remember, you have to ask for your personnel file,mand your medical file, as separate items, on the same eVetRecs request, OK?

Also, if you used any VA facility after you got out, you'll have to get your stuff from each VA facility separately possibly, VA, I dont think, consolidates all clinical records, at least my stuff wasn't, so you might have to call each place and its ROI ofc individually, OK? However, that's VA, not the svcs, all that suptuff positively goes to NPRC, that I know for certain, OK?

Look up the term eVetRecs for NPRC in St Louis...by now, your stuff should all be there, do the Java applet, there isn't much room, you might have to be creative to fit everything, or do a cover letter in addition, just follow the instructions, trust me, then print out the sheet, sign and date it, and fax it, along with a cover letter if you do one, to the NPSC fax number listed, just dont lose it, or the request number, or youre gonna have to start from scractch, explicitly ask for your personnelmfilez and your !medical file, you can get both, you'll just need to ask explicitly for both sets...thenz wherever you were assigned, call the facilities involved, and get the med record unit for the clinics or hospitals involved, though NPRC should have most of it...of you used a VA facility while on active, those won't ne there, probably, you'd probably have to call rthe release of info (ROI) office for any VW facility to get that stuff, though NPRC should have all your strictly active duty med records, my wife and I were able to get all of mine doing what I just explained, if you have trouble, or are wondering what's going on, just call the NPRC phone bacpnl, though you MUST have the request number, MUST...it'll take a couple of weeks for them to send you a notice they're looking, then about a month or two probably for your stuff to show up, it might ne double sided, that's just how they do it to save paper, so number all the pages it's really easy to get them all out of order, trust me, been there, done that (BTDT), OK? If you need more help, ask, I'll try to suggest more, trust me, that's how you get the!, honest, we did it, we know, honest, OK?
