Is there an example for a DA Form 4187 for someone trying to be considered for the upcoming SFC Board that has over 18.5 years?

Is there an example for a DA Form 4187 for someone trying to be considered for the upcoming SFC Board that has over 18.5 years? I was told you can still get looked at but you have to get documentation (4187) that you are within good standing with the unit and it is approved by your chain of command? Can anyone help me out here or should i just hang it up and retire and not try to get promoted???
