I believe it depends on what state you are in. Despite being the breadwinner in the household, I still had to file jointly with my wife so both our names appeared on the deed to the house.

It is your choice if you want your spouse on the loan or not. When I did mine, we left mine off because she wasn't working at the time and we didn't want her debt calculated into it.

A good loan officer will take a look at both credit scores and advise the path most beneficial to you. Many times the spouse boosts the score, hence a good idea to go joint.

No, no requirement. Our first house , 1st VA loan was all on me, second house all on my wifes VA cert and loan, 3rd house was my second VA cert and a joint loan.
