Go to the VA Or Veterans Center in your local major city that has one and theyll either give you a form so you can request one from the department of defense or from your local recruiting office. A army recruiter helped me re order mine.

PFC Eric Bast Did you submit a VA claim? Log onto eBenefits and go to your documents. That should have a copy.

The other idea is to go to the NPRC website and request a copy of your military records.

Did you register with a state/county Vet Center when you ETSed? They may have a copy if you gave them one. Many counties do this for your exact reason.

Did you make a digital copy send it to a potential employer? They may have it. It might be in your sent items.

If you haven't been out a long time (under a year), you could request one from the installation that out processed you.
