The unit runs it. It's the responsibility of the medics to request the class through AMEDD and conduct it. When your unit needs to deploy everyone will get the class.

Don’t worry, it’s required if your on the line. At least it used to be. Command should have it annually on their training schedule.

SPC (Join to see) the requirement is in AR350-1. It was 1:10 or one per squad. At Campbell, 100% was the requirement, plus four additional tasks to be Eagle First Responders. The ARNG might be a little more difficult. Tell your first line you want to be one. They need one to run ranges usually.
