In the Army alone it would be General of the Army of the United States George Washington

Commander In Chief? or would is be 5 star general who is a direct presidential appointee and only during a time of declared war, ie Eisenhower

Nothing is higher than the dependant wife of a Staff Sergeant!! LOL. Actually the president does not hold any military rank,even though he is at the top of the chain of command. And the 5-star ranks were implemented in order to give Eisenhower and others and equivalent rank to our allies. There hasn't been one in over 60 years, even though it's technically still authorized by regulations.
