This one made it through two reviews before someone caught it:
"Works extremely hard at meeting the low expectations he sets for himself."
It was not in my Chain of Command but ended up making the rounds in the battalion after it was caught. Got a great laugh out of it (name and other identifying info was blacked out, of course).
