What is the "Reserve Component Individual Acknowledgement of Dwell"?

I am currently working on a packet to submit for a Tour of Duty position and it says that I need to sign a "Reserve Component Individual Acknowledgement of Dwell or Notification of Mobilization form dated 5 February 2014" Has anyone heard of these and know where I get them?

Sir, if you have dwell timing remaining you have to sign a waiver before you can mobilize. Once you sign the waiver you cannot revoke it.

You should be able to use a 4187. Your S1 should be able to provide you the help you need to complete your TOD packet. Also, there should be a POC for every position in TOD that can provide assistance.

Yes if you have been deployed before as a reservist I believe you get 2 years of stabilization time which is what they are calling dwell time before you can be deployed again.
The form should be given to you by your unit administrator or the person handling you packet at the Pentagon DCS G3:5/7 section as they are the ones in charge of the Tour of Duty slots.

That one turned out to be just a 4187 with "Waive Dwell Stabilization" typed into the "other" section. That might be it but I am not sure.

Yes that is the form they are using now because when I did a tour of duty mob they had me to type a statement sign, date, and have it notarized to be added to my packet because at the time I was IRR and did not have a UA.
