SM lives in barracks because SM is not married.
SM stays off post with their partner.
SM usually packs required uniform(s) for the normal duty day.
SM shows up for First Formation in civilian clothes then changes to uniform that is put out before formation.
SM forgets to pack both long & short sleeve PT shirt.
SM is then required to wear duty uniform and miss PT.

They can do pt in uniform and be sweaty the rest of the day. Make sure they wouldn’t want to “forget” pts again.

I, personally, would make them show up early for a while, already in the proper uniform.
When was the uniform posted?
Why is this soldier showing up in civilian attire?
Can't the soldier bring OCP and full PT uniform every time?
This soldiers social life may be interfering with their readiness.

I always put it in writing. Paper trail is important if they create a habit of it. It also demonstrates your desire for their change, and safeguards further actions that you take. Counseling's can quickly become legal documentation.
Always ask yourself if this could also be a symptom of something else going on in the soldiers life. Ask them questions about other things to gain a full perspective of their weekend/ financial/ relationship/ etc. and the stress that may be present. Don't pamper them, but be aware.
Worst case scenario is a soldier that is going through some behavioral health issue that are borderline suicidal. ASK! BE TACTFUL!
I also sit down with the soldier and have them help with the plan of action. This helps them know why you administer certain actions. It its "their idea" then they wont fight it as much.

GOOD Grief- Is the SM normally squared away?. NO Sm is forced to miss PT unless they have a limiting profile. You can do PT in uniform. They can be forced to live in the barracks until the Coc feels they have atoned and learned. They can have their weekends filled with inspections,layouts etc. Until the point is made as corrective action. an NCO is own limited by the depth of their thinking.

What kind of counseling would be written? What Plan of Action would be just?

Has this happened before? Is this Soldier normally squared away? Is SM a PT horse or a minimum standards type?

There is no cookie cutter answer to these types of questions. You as an NCO are given the leeway to use your judgement.

To be honest with you, if it's the Soldier first time forgetting to pack his full uniform, then I would just give him a verbal. If it was his 2nd time doing so, then it would go on paper. Not being in the proper uniform..... the plan of action would be: have SM sign in at CQ 20 minutes prior in prescribed uniform and if push comes to shove, restrict Soldier to barracks room, since he forgets things at his 2nd home.... Ensure when you put the magic bullet, you research the actual regulation and chapter...... that particular chapter may not correspond with infraction at hand......

They can do PRT in duty uniform. That will certainly ensure they don't "forget" to pack the proper uniform. Or at least....call someone before departing for base to find out what the exact uniform is. Or....find out what the uniform will be for the following day prior to departing base at COB. Team Leader could always take the initiative and ensure that this Soldier is informed at all times since the TL knows they are living off base even though they have an assigned barracks room. Speaking of that....did the CoC bless off on this SM living off base?

The initial counseling should state what time and the pt uniform. Once SM signs that showing that SM is aware of the pt policy and hours are, the next step would be a negative counseling explain the corrective action and what would happen if SM failed to comply. I hope that helps. I work in an orderly room. Once you have a packet of negative counseling, the next step is an ART 15 company grade.

I understand that you are in the states, but don't forget the bad guys do target Soldiers in uniform off base. The Soldier forgot to pack both long and short APFT shirt.....

SSG Laurie Mullen there have been a few high profile cases from shootings to bombings involving military personnel or facilities in the US. Remember the bad guys are watching....
