What should I put for Part IV Duties and Responsibilities for my OER Support Form?

I am a C Med Evac Platoon leader in the guard. I'm trying to figure out where, either I can find all the duties and responsibilities with estimated monetary values and such, or if there is a template somewhere I can steal.

I remember the days in which I would do an OER Support Form at the beginning of my tenure. These were the old paper days.
Personally, (1) I would ask the Battalion S1 for samples if they have them. (2) Do not be afraid to ask your rater for assistance. After all, he/she is supposed to know what you are to be doing. (3) Consider asking the officer you replaced for their Duties & Responsibilities.
