I've been dealing with a pretty serious back issue for awhile now and have been going to physical therapy for it to try and get it fixed. I was wondering what documents I'd need to provide for non-military related medical organizations to fill out to potentially list out details of what I am dealing with and limitation recommendations so that I do not make it worse when I am at drill.

You're looking for a DA Form 7809*. Once filled out you need to give it to your leadership unit leadership and it has to make it to a medical profiler, and who that is is will vary for you depending on which unit/state you are in. If nobody is familiar with who that is, contact your State Surgeon's office.

Get a letter like the provider would write to Workmans Comp or an employer on practice letterhead with a signature of the provider and then take it to your medics and have it put in the military system.
