What would be my punishment if I couldn't return to my post to sign my DD214 due to medical issues and finance issues?

So my current day out of the active Army was September 17th 2018. I left the military base on September 5th due to my daughter being sick my depressional state I was admitted into the hospital for depression twice and my current wife situation. I had a schedule court date for my daughter as well a letter from her doctor and they chain of command wouldnt allow me to leave early. I cleared everything on post the only thing that wasnt donr with sign my DD214. I wanted to know if I couldnt return to my post due to medical issues and finance issues what my punishment or where they take it.
