Has anyone here made up the 4187 and MFR for a compassionate reassignment? I'm currently building up my packet for one and no one in my COC has ever messed with one. The only person who has dealt with it is my PLT SGT and it was nearly 6 years ago. Any help would be appreciated. I have no idea what either form should read so any input or templates are welcome.

Your best bet would be to reach out to a chaplain. They have dealt with many of these requests. Also, ensure you meet the requirements for the action.

I apologize for the delay, I’ve been out on emergency leave. Provide a good email address so I can send you an example.

I believe AR 600-14 has a sample form in one of it's figures.

There is no 4187 or MFR for compassionate reassignment. It has it's own form. Google compassionate reassignment HRC, and look in AR 614-200
