Where can I find an example of how to fill out a 4187 for paternity leave?

I am trying to take paternity leave and my leadership doesn't know how to fill out the 4187 and I can find any examples on line.

I just went through this! It’s a 4187 to make you a secondary care giver to your spouse. The 4187 allows 21 days of paternity leave. You still need a leave form and on the reason you put care giver instead of paternity leave. My OPS had a format and you turn in discharge paperwork for baby and they handle the rest.

Why would there be a 4187 to request leave? We have a leave form for that.

PV2 (Join to see) in that case, you may need to ask to defer your report date as well, unless your report date is after that five weeks. It usually isn't that long for IET Soldiers

My report date is April 24th, that’s why I figured taking leave would make sense since I’ll be a hold for 24 days. SFC (Join to see)

If it's your kid all you need is the DA 31, the 4187 is if you are assuming duties to become the primary caregiver.

To retroactively take the additional 11 days of paternity leave, I had to fill out a 4187 designating myself as a secondary caretaker. After reading through former sec. Espers directive about creating the 21 day paternity leave, I was not under the impression that taking the leave post directive would require the 4187 still. However, that was 4 months ago that I took the retroactive days, so that very well may be the new standard for taking paternity leave.
