Where can I find an example of the 4187 needed to request a deletion of orders?

Or more information on swapping orders with someone (already have someone willing to take my place). I'm about to get divorced and have a 1 month old baby and orders to PCS to Korea with my soon to be ex. I already talked to my branch manager and he said to submit a 4187 with justification for deleting the orders. My biggest concern would be having no FCP while over there.
