It's issued at CIF or RFI with your body armor or rifleman kit.
If you lost the one you were issued either acquire one off post at a surplus store. There's more than one version so ensure you get the correct one. Your other choice is to fill out your statement of charges and get it reissued.

If you're looking to build a personal one, Amazon.com has a wide selection. If you're looking to replace Army issue kit, Ebay has the IFAC Pouch and refill kits. I loaned out my bandoleer once, as well as left a black fleece laying on a cafeteria table. Each one has been replaced by Ebay.
