Who should I contact at my gaining unit to fill a DA Form 5434 (12P School)?

Should be your sponsor. I just got assigned as a sponsor for an incoming SSG to my unit. He told me he needed me to fill in his 5434 on ACT but that's the first time I've ever seen anything like that done with PCS. I didn't use ACT when I PCSed here.
If you don't have a sponsor your next unit is failing you because you're supposed to get assigned a sponsor. My person isn't reporting until July and I got notified a month ago and they are reporting early.

Thank you for your input SFC Fuerhoff. Yes, I know it's supposed to be my sponsor. I've yet to be assigned one and it's a school for 12P so I know there are many like me going there, so I don't know if they have a single POC for sponsorship or what for incoming students reclassing. I've called the school itself with no answer today. I'll be calling again until I get one. I posted on here to see if anyone has more experience with it and could advise who exactly I should get a hold of. I too have never had to do this before and this is my 4th PCS.

Hmm. I don't know then. I never have dealt with the 5434 until I got assigned to someone incoming last month. I guess keep calling the school and maybe tell whoever is in charge of you and see if they can get any traction up the chain. I didn't even get anything in ACT so I just emailed the information to the incoming soldier.

You do not contact your sponsor (unless you personally know them) because the gaining unit's TASP (Total Army Sponsorship Program) Coordinator is the one assigning your sponsor, now he/she will get an email saying fill out the Section 3 of the 5434. But You will also have to go to https://actnow.army.mil and fill out everything without leaving a blank space meaning put N/A or None in the rest of the blank spaces or else it wont count as completed.

Not always. I just got assigned as a sponsor last month. I never got anything in ACT and this is the first time I've ever seen PCS done like this. It might be because I'm in a joint unit. I had to email the soldier the information for the 5434 and that worked.
Someone should have gotten a hold of this soldier by now if he's clearing. So they either didn't assign a sponsor or something fell through the cracks.

It's actually the posts, if they allow their Soldiers to PCS without going through TASP then the Soldiers will PCS no matter what, some posts will not publish your orders until you've been assigned a sponsor or have put in the 4187 for ETP for not having a sponsor.. The Army has had this program for a long time but just recently is when it's being more implemented. If you never got anything from ACT it means your "name" wasn't assigned in the system to be a sponsor or else it would've been automated as soon as somebody did - gotten the email and instructions
