How do you properly fill out a 4187 for an assignment swap and who is it routed through ?

There's no proper way to fill out a 4187. There is absolutely no regulatory guidance about the 4187 other than saying it's the form you will use for personnel actions.
However, there is regulatory guidance about every single thing you ask for including assessment swaps and you should reference those things on your request. For instance, for a swap you should refer to the Soldier SSN and name who has volunteered to swap assignments with you and you should reference and acknowledge the portion that states you are responsible for paying for all travel expenses and that the person you are swapping with is willing to accept that they are willing to personally pay for all travel expenses incurred with moving and that they will not be eligible to submit a travel voucher, they will pay for the shipment of their household goods, their family, they will receive no TDY or lodging expenses, DLA or mileage expenses.
If you find someone of the same grade and promotable status who is willing to trade assignments with you and pay for their own PCS, the PCS of their dependents, and the cost of moving their own possessions, then you stumbled upon a unicorn.

Back in the day, those Armytimes Stateside swaps were so flaky. We had a soldier do a,swap with someone for Ft Irwin, and then elected not to show up. They went and signed in somewhere else and my command got monkey pumped. Our guy had already went on his PCSed way when we figured it out. We dug into it and our 1SG figured out that there was no way to compel big Army to fix it and it was "on us" for allowing the move. From there out it seemed that the state side swap was DOA. Every 4187, recommended disapproval. Eventually they just went away.
I never understood why the Army simply allowed soldiers to disobey PCS orders and "sign in" somewhere else. Why that was allowed to continue I have no idea. Especially NCOs. Should have been fixed with GOMORS. Gee, you don't want to go to FT Polk? Why didn't you say so. Just pick a place and go....said HRC/PERSCOM never.
Only time I ever saw that work is before they departed losing station, both soldiers (same grade and MOS) went to see their mutual commander and swore they would switch, 4187s, then the orders were rescinded and reissued to reflect the switch. Both gaining units got their 10 level soldier and life went on.

LTC Jason Mackay that's why eMilpbamd EDAS drive assignments and reporting now. Gone are the days of showing up and signing in to another unit, if they try that they either get redirectes or reported AWOL by the command that is supposed to receive them.
I remember the Army Times swaps back in the day. That system is completely dead now. What replaced it is something where both members have to sign a statement that says they are responsible for all costs of PCS and will not hold the government responsible for any expenses incurred. There's not a lot of Soldiers willing or financially able to pay for their own PCS.

Your S1 will/should have a template. Bear in mind that the following need to happen:
1) There must be someone of the same grade and MOS as you that is also looking to swap to your present location.
2) They, too, must fill out a 4187 request.
3) Both their Commander and your Commander must sign approval on each SMs respective request.
3) The completed requests runs thru the S1 and CoC channels
If either Commander says NO, then the SWAP is not happening.
