ANNUAL WORK SCHEDULE: Class 5, 6, or 7 Non-exempt

DAILY WORK SCHEDULE: Varies based on the needs of the Division

ESSENTIAL FUNCTIONS:

Perform all delegated nursing services as outlined in the individual’s task analysis.
Provide assistance with meals and personal care.
Engage members in community activities and volunteering.
Responsible for completing all required documentation.
Responsible for following all programs as outlined in the member’s ISP and Behavior Support Plan.
Ensuring all services are provided in a timely and efficient manner in accordance with each individual’s ISP.
Participate in team planning meetings as needed in conjunction with the Supportive Living Coordinator.
Ensure the health and safety of all individuals.
Maintain a clean and safe living environment.
Assist with daily living tasks – grocery shopping, meal preparation, transportation to appointments, recreational activities, etc.
If necessary, fill in as an alternate at various worksites or other divisions.
Uphold the rights, dignity and respect of all members.
Other duties as assigned.
QUALIFICATIONS EDUCATIONAL: High School Diploma or GED

EVALUATION CRITERIA: Position will be evaluated annually based on the Performance Evaluation Matrix

QUALIFICATIONS REQUIRED SKILLS AND OTHER:

Must be at least 19 years of age.
Must have a valid Ohio Driver’s License and good driving record.
Must pass pre-employment and random drug screenings.
Must pass a BCI background check.
Must remain in compliance with all regulatory standards governing program services, including appropriate trainings.
Must not be a registered offender on the ODDD Abuse and Neglect Register or the Ohio Department of Health Nurse Abuser Register.
Must have reliable transportation and your own auto insurance (to conduct agency business in personal vehicle and receive reimbursement employee must have the amount of 100/300/100 coverage).
Must have or obtain First Aid and CPR training certification.
Must be able to communicate in a positive manner utilizing both written and oral methods.
Must be able to lift at least 50 pounds on a regular basis.
Must be able to work a varied schedule, if needed.
Ability to assist individuals with challenging behaviors through implementation of supported plans.
