This position is for a PACT LPN with the Huntsville Clinic

The Licensed Practical Nurse (LPN) works under the direction of a Registered Nurse/Nurse Manager with Primary Care Services (PCS). The LPN is responsible for observing, recording, and reporting changes in the patient's condition as well as assisting in the development and revision of the patient-driven holistic care plan for life. The incumbent provides a wide range of clinical and technical services to the Veteran under general supervision of the Registered Nurse. The duties include checking in the patients, resolving appropriate clinical reminders, patient education, and assisting at the front desk. The LPN demonstrates active collaboration and communication with other members of the Primary Care Service. Additional technical and administrative support and other support duties as assigned.
