Does anyone have a sample memorandum for justification to go back to IRR?

I recently came into the AGR program(USAR) and I am trying to get into the IRR now for Cogent Personal Reasons like marriage problems and I was told I had to put it into a memo, I'm not good at writing so if anyone could help me out I would really appreciate it.

I would recommend that you go and speak with your retention NCO and they would probably be able to guide you or even assist you with filling it out.
