We discussed the importance of reducing employer/employee negligence in one of my classes recently. This is usually why, but you cannot prevent people from sometimes just being reckless.

I am never understand of how government agencies can get away with allowing employees to work doubles. I have done it. There were times I considered it a day off when I had to work only one shift. It effects good judgement and reaction times.
Pretty sure this guy is on the hook for manslaughter. As a cop I have seen some terrible driving practices with emergency vehicles. Police officers get to the point they think of themselves as invincible and put the same people they are sworn to protect at risk. It is a culture that is cultivated. Policy says one thing, but supervisors allow something else. Most government agencies believe they are covered if a police officer goes against policy. There is a legal prescience that says if the company has a policy, but allows it's employees to act in a manor that is not compliant then that policy is not in effect. USC is on the hook for a civil suit too.

With me, I always saw it as a bit of both. Most basic police training is standardized in the US. With the exception of the state law portion every accredited police academy has the same standards. Where it starts to go sideways is from the field training officers to supervisors. As a FTO it is really the first chance to take the skills learned at the academy and apply them to the real world. FTO programs very with each department. The culture in each department is different. My first job out of the academy was a little Podunk town with 6 officers. I was given a set of keys showed where the city limits were and turned loose. Luckily I had a State Trooper that took me under his wing and taught me things that would keep me alive. My last department I worked at (as a police officer with 9 years under my belt, was a FTO and a SWAT officer) I was under a FTO for 4 months.

Wow, this is very distressing.
Of note, USC is private not public, so their security folks would be as well, not public police like LAPD...
