The U.S. military's stats on deadly airstrikes are wrong. Thousands have gone unreported

This sounds bad....but could someone tell me what other country goes to war and tells the world and their enemies exactly what we are doing, and then say we screwed up a database?
Do active duty know the word OPSEC? Do leaders know the word OPSEC? Where the heck is the Counterintelligence personnel, in USAF, that is OSI.
