how are these hackers able to breech security? flaws in password protection or system failures with software?

You are presuming, SGT Jim Arnold that they had a properly configured database with security in place to protect it from being accessed. Earlier this week I read about a breach where proprietary data was taken because the hacker had a client Username and Password.
