Firearms training should always be done with well trained instructors in absolute control of the class. Any deviations or goofing off incidents have to be extinguished or banished immediately, or else stuff like this happens.

Dry firing with rounds? That's how you get negligent discharges and investigations -- They need to re-train the trainers on basic marksmanship training
