Sometimes you have to consider the propaganda is being worked on both sides of this. The South Korean Government is perhaps a bit more believable. There was also some testing done on other defectors of the years and similar parasites were found in many of them as well. I believe that the issue, along with the famine that struck the Norks in the 90s can be traced back to the fall of the Soviet Union.
Without assistance from the Soviets the Norks were no longer able to produce fertilizer for agricultural needs, and production plummeted. They went back to the old school method of fertilization...... using human waste. Human waste works, but also is a breeding ground for parasites..
That a soldier who is stationed in what should be the most important posts in the nation is in such poor health leads me to believe the vast majority of Their Army is in similar or worse condition.
