If he was declared as not completing a requirement for O6/COL then why he didn’t got demoted as his last qualified grade/rank? Just an observation .

My thinking is he now has 17 years of experience post-schooling. IF his generals and instructors/trainers didn't catch it, aren't they at fault.? He doesn't set the rules; they do. My MHW3 in charge of training will rip up the paper if she can't read your signature, and you will resign it until she can read your name. She will tell you this is an official state document that goes into Boston and have you redo it until she's happy. You will sit there as long as it takes.

William Barry, there is so much blame to give at the school house - his instructors and whoever is in charge of QA/QC not at the command element. But my statement is a rhetorical one although it would be nice if in fact actions as such take place.

MAJ Javier Rivera - My MHW 3 is in charge of training caregivers for the mentally ill. Many times our clients are suicidal. If we make a mistake, they can die of an overdose, hanging, etc. It is a huge responsibility, and I have seen her rip up or throw out a paper just because it is not legible.

I wonder what caused the Army War College to convene an academic review board after almost 20 years?
'watchdog investigation'?
'watchdog investigation'?

Wow. That's a long time to go only to have someone dig in your past academic work. I wonder who/what he plagiarized.
